All appointments including After-Hours Urgent Care are for our rostered patients only and must be booked in advance as an appointment. There are limited same day spots that can be booked during the day. Appointments can be made online or by calling us during office hours.
Yes!
We have a limited number of same-day appointment slots available with other practitioners if yours is not available, including our Physician Assistant. There are also resident physicians who may have open appointment slots during the day.

If your imaging has been completed at a local hospital (Palmerston, Mount Forest, Fergus, etc) you can now obtain a copy of your records through Pocket Health. For more information please visit www.pockethealth.com/patients or contact the PocketHealth Patient Experience Team at 1-855-381-8522.
If your OHIP is expired or due to expire soon, please visit this link for more information on how to renew your OHIP card. It is your responsibility to ensure that your OHIP coverage is kept up-to-date at all times.
Any paperwork, forms, insurance note requests received by our providers takes a minimum of two to four weeks to complete. You will receive a phone call from our business/billing department once the forms are completed and ready for pick up.
Lab requisitions are valid for 6 months. For non-urgent bloodwork, you should wait 1-2 days after the requisition has been faxed to the lab prior to getting your bloodwork done.
We continue to experience a large volume of calls, please be patient and try your phone call again if needed.
Referrals get sent to a specialist office from your family practitioner. Once received at the specialist's office the referral will be triaged according to urgency. Wait times vary from office to office. When the referral is triaged either by the specialist office or the family practitioner office will contact the patient.

Check the label of your medication bottle. If the label shows a number next to the "refills" section, that's how many times you can get more of your medicine without going back to see your healthcare provider.
In Person. Go to your pharmacy and ask them to fax a prescription refill request to our office.

Once our office has received your request, your healthcare provider will review your prescription, and if appropriate, will sign off and send it back to your pharmacy for dispensing.
Your healthcare provider will review your refill request once received from your pharmacy. We require two weeks notice to process prescription renewal requests so please plan ahead and do not call our office until it has been at least 14 days after your initial request.
