While OHIPcovers most medically necessary physician services, there are some services that are not covered by OHIP. These are known as uninsured services, and patients may be responsible for the cost of these services.
Examples include certain forms and certificates, driver's medicals, transfer or copying of medical records, some cosmetic procedures, and other services that are not considered medically necessary or are requested by a third party.
Our fees are based on guidance from the Ontario Medical Association (OMA) and are reviewed periodically. We make every effort to keep our fees reasonable while recognizing physician time, administrative support and other costs involved in providing these services.

Please Note: Fees marked with an asterisk (*) are estimates based on the time typically required to complete the service. The actual fee may vary depending on the complexity of the request and the physician time required. Patients will be advised of applicable fees before the service is provided whenever possible.
