Dr. Deacon started working with the Mapleton Health Centre in 2013. His favourite part of being a rural physician is getting to know his patients and their families as people and helping/working with them through all of life’s ups and downs. He also enjoys supervising medical student training through McMaster and other Universities.
Dr. Deacon is the Medical Lead Physician for the Minto Mapleton Family Health Team (MMFHT), a board member of the Guelph Wellington Physician Association (GWPA), serves on the Medical Advisory committee with North Wellington Health Care and is the FHT board representative on the Guelph Wellington Ontario Health Team (OHT) Governance Council.
Outside of work Dr. Deacon enjoys running, traveling, and spending as much time as possible at home with his wife and three young children.

Dr. Peterkin started working at the Mapleton Health Centre in an official capacity in December 2003 after doing a summer locum for a month. She went into Rural Family Medicine because she loves the idea of being part of a smaller community that she gets to care for and grow right along side with.
Dr. Peterkin is the president of the Professional Staff Association for North Wellington Healthcare where she gets to advocate for her peers at various tables from the Hospital Board to the Guelph-Wellington Ontario Health Team. She is also the newly appointment Chief of Staff for NWHC. Her other Board Affiliations include the Minto-Mapleton Family Health Team and the Guelph Wellington Physician Association Steering Committee.
In her spare time Dr. Peterkin enjoys writing poetry, singing, dancing, baking, walking in nature and spending quality time with her family and fur babies.

Dr. Sun obtained her undergraduate science degree at Queen's University and completed her medical degree at the University of Ottawa. She has been working with the Mapleton Health Centre since 2012. Dr. Sun loves working in primary care because she gets to follow her patients longitudinally. She finds it very rewarding to use her medical skills to help her patients whether they are an infant, adult, or senior. In particular, she has found that working at Mapleton Health Centre is amazing because her patients really value her opinion, and she has a supportive team of colleagues and staff.
In her spare time, Dr. Sun is usually busy doing activities with her young family or plucking weeds from her garden.

Nurse Practitioner

Jodi started working with the Minto-Mapleton Family Health Team in 2009 as part of the "Grow Your Own NP" program. She enjoys working in primary care, really getting to know her patients and their families and support people in times of vulnerability and finds helping them achieve their physical and mental health goals rewarding.
Jodi is part of the FHT Quality Improvement Committee, OHT Quality Improvement Committee, and Regional Children's Mental Health Committee. She is part of the MMFHT Complex Care Clinic and is a Nurse Practitioner Association of Ontario Board Member.
Jodi is a proud hockey mom, sewing enthusiast, wannabe flower farmer, and self-proclaimed food critic.

Sam started working with the Minto-Mapleton Family Health Team and the Mapleton Health Centre in 2020. She obtained her undergraduate degree, Bachelor of Science in Nursing, from the University of Western Ontario in 2012. She then went on to complete her Master of Science in Nursing, Nurse Practitioners, at the University of Toronto in 2019.
Sam's favourite part of working as a rural Nurse Practitioner in primary care is getting to know her patients and their families and working with them to achieve their health goals. Sam also enjoys working as part of a team, alongside the many knowledgeable providers at both the MMFHT and MHC.
In her spare time Sam enjoys reading, gardening, hiking, biking, yoga and spending quality time with her family.

Patrice started working with the Minto-Mapleton Family Health Team in 2019 and joined us at Mapleton Health Centre in 2022! She obtained her undergraduate degree, Bachelor of Science in Nursing, from the University of Western Ontario in 2007. Since then she has worked in a wide variety of settings over a course of 15 years including general surgery, orthopedic surgery, trauma unit, and interventional radiology. She completed her Masters of Nursing, Nurse Practitioner, in 2019, specializing in Primary Care.
Some of her favourite things about working in primary care include making a positive impact by providing holistic, accessible care to individuals and families in our community as well as providing preventative care to enhance welling being for our patients over a course of a lifetime.
In her spare time, she enjoys travel, being outdoors, yoga, and trying to keep up with her husband, 2 daughters, and fur baby!

Cailin Hill is a Physician Assistant who completed her Masters in Health Sciences at Duke University Medical School. As a PA, she is an advanced practice health care provider who works as an extension of her supervising physicians. She performs histories, physicals, orders and interprets investigations, diagnoses, formulates treatment plans, and performs procedures. Cailin believes primary care is the foundation to good health and enjoys optimizing the care that patients receive.
Upon graduation from her PA program, Cailin followed her Canadian husband north to be part of the Ontario Physician Assistant Pilot Project. After having her first child, Cailin took on the role of Physician Assistant with the Minto-Mapleton Family Health Team in 2011. Born & raised in rural Pennsylvania Dutch country, Cailin recalls visiting Drayton for the first time as feeling like coming home.
Outside of work, Cailin enjoys reading, hiking, gardening, board games, knitting, and baking. But as a mother of 6, you can more often find her doing laundry & dishes.
Beyond the home, Cailin serves on the Interprofessional Healthcare Provider Advisory Council for the Association of Family Health Teams of Ontario. She is also the Networking Co-Lead for Physician Assistants in Waterloo-Wellington region.

Sylvia started working with the Mapleton Health Centre in 2019. She enjoys working in primary care because of the variety of work and caring for people of all ages. In her spare time she can be found outside, camping, and caring for her many pets and farm animals, including her asthmatic donkey, Stewie!

Makayla started with the Mapleton Health Centre in 2021. Her favourite part of working in primary care, by far, is creating connections with patients and other allied healthcare workers. The best part is watching patients go through the different stages of life and being a part of their journey. And, of course, she loves the babies! She says that everyday is different in primary care and it is always busy - that's what got her into their field and why she feels she will always stay.
In her spare time Makayla enjoys traveling, reading, being outside and spending time with friends and family.

Sarah started working with the Mapleton Health Centre in 2021. Sarah is a jack-of-all-trades and spends her time equally between the front desk and the nurse's station. She enjoys the flexibility and variety of working in primary care.
In her spare time, she enjoys yoga, listening to audiobooks, and spending time with her family and friends.

Trish started working with the Mapleton Health Centre is 2010. She started out primarily doing reception work and slowly learned OHIP billing and she is now our lead Billing Clerk. Trish enjoys working in healthcare because she has always enjoyed helping others and tackling new tasks. Trish has a Diploma in Administration and a Bookkeeping Certificate. She’s a numbers girl!
In her spare time, you’ll find Trish running away to her family cottage in the Muskoka’s, baking, and spending time with family and friends.

Ingrid joined the Mapleton Health Centre team in 2014. She says that healthcare is always challenging us but working with such a great, supportive team is fun and rewarding.

Kim started working at the Mapleton Health Centre in 2021. She enjoys helping those in need and if she is able to make navigating their experience with the healthcare system a little easier, it makes her happy. She enjoys the fast-paced environment and that each day is a little different.
In her spare time you can usually find her playing baseball, skating, camping, reading, crocheting or spending time with family and friends.
Receptionist
Julie started working with the Mapleton Health Centre in 2023 after the physician she was working for within the Family Health Team moved to another area. She has worked in healthcare her whole life as a massage therapist for 16 years and eventually transitioning into medical reception. She enjoys the process of helping people feel better.
She spends most of her time at the arena during the winter months watching her two daughters play competitive hockey and spends her summers on the ball diamond coaching and watching her daughters play ball. She also enjoys spending time riding her horse.

When Kim started work at Mapleton Health Centre in 2019 she had recently retired from the Waterloo-Wellington LHIN after 22 years. Working in healthcare for most of her life has taught her that navigating the system is not always easy, so she is always trying to help patients have a smooth journey. She loves working in the front office at MHC, seeing many familiar faces and meeting new people.
In her downtime Kim likes to spend time with her family (including her 5 grandkids and 3 grand dogs). If the kids don’t keep her busy enough, she loves to read and do a little crafting with her Cricut.
Clinic Manager
Elizabeth started with the Mapleton Health Centre as a co-op student around 2007. In 2015 she came back as a nurse. In 2021 she took over as Clinic Manager and continues in this role today. Her favourite thing about working in primary care is getting to know her patients and being able to see and be a part of the progress they make over time.
Elizabeth is a member of the FHT Quality Improvement Committee, the Integrated Primary Care Team Internal Working Group, and sits on the Board of Directors for the Ontario Medical Group Management Association (OMGMA).
In her spare time, she enjoys spending time with her family at the lake, trying hard to be a gardener and always learning new things.
