Non-Urgent Requests

This online portal is for massage, physio, chiropractor, or orthotics only.

Please be sure to include the following in your request:

What type of prescription you are requesting (i.e., massage, physio, etc.)
Why you require the note/prescription (i.e., back pain, neck pain, plantar fasciitis, etc.)

Things to Know:

Turnaround time for these requests can be up to 14 days.
There is a fee of $20 per note requested.
You will receive a call when your note is ready.
Payment is required BEFORE you receive your note.
Requests for anything other than those listed above will not be answered.

If you are experiencing an urgent concern please call into the office for assistance.

If you are experiencing a medical emergency please go to your closest emergency room or call 911.
